
Application Process for Charted Way Foundation Respite Program
To apply for participation in the Charted Way Foundation respite program, families must follow these steps:

Submit the Inquiry Form: Complete and submit the inquiry form. This form provides Charted Way Foundation with initial information about the family.

Complete the Online Application: Upon receipt of the inquiry form, Charted Way Foundation will send an online application to be filled out by the primary guardian, caregiver, or the individual seeking respite services.. This application includes a doctor’s release form, which must be submitted with a doctor’s approval for all applicants currently receiving treatment. Bereaved families are not required to submit the doctor’s approval form.

Application Review: Once the completed application and, if applicable, the doctor’s approval form are received, Charted Way Foundation will review the application. The foundation reserves the right to determine the best fit for families based on available respite options, compliance with all guidelines and eligibility criteria.
